• Live registration certificate of Chhattisgarh Nursing Council
• Live employment registration of the concerned district (Bastar division)
• Residence certificate of a Bastar division district
• Identity proof (Aadhaar, Voter ID, Passport or PAN card)
• NOC from employer, if working in a government/semi-government institution
